On September 16, the Spanish drug-checking service Energy Control updated an alert after detecting paramethoxymethamphetamine (PMMA) alongside MDMA in two pills sold as MDMA. Both samples reached their services after being purchased on the dark web. The organization warns that the source does not guarantee they are limited to that channel: once inside the unregulated market, they can end up anywhere.
Two samples, a European context
Energy Control is explicit about what it knows and what it does not: two detections are not enough to determine the extent of the phenomenon in Spain, and the September notice stated that no additional cases had been identified by that date. The reason for publishing the alert is the broader context. In recent weeks, several European drug-checking services have found PMMA in pills sold as MDMA, sometimes mixed with it. In a market where batches cross borders easily, those pills can reach us here as well. A few weeks ago, we reported on a red alert from the Dutch DIMS system regarding a pill containing a very high amount of PMMA.
Why PMMA behaves differently
PMMA belongs to the amphetamine family and is chemically similar to MDMA, but its effects and risks are not the same. It increases available serotonin while simultaneously hindering its breakdown, causing an excess to accumulate that can trigger serotonin syndrome. MDMA also elevates serotonin, so the combination multiplies that risk. The other issue is timing: the effects of PMMA can take longer to appear and are not what one expects from MDMA. Someone who thinks the pill “isn’t working” tends to take more, and that is where severe intoxications occur.
What this implies
Energy Control recommends not consuming pills mentioned in alerts and adds four guidelines applicable to any similar situation: do not trust appearances, as color, shape, or logos reveal nothing about the content; do not redose if effects are delayed or different than expected; avoid mixing substances; and test your substances whenever possible. Regarding warning signs, the service points to very high body temperature, intense agitation or confusion, a very high heart rate, muscle rigidity, seizures, loss of consciousness, and any rapid deterioration: if any of these occur, call 112 as soon as possible. In cases of hyperthermia, every second counts: seek urgent medical care and tell healthcare staff which substances were consumed. This alert illustrates the value of this infrastructure: the same service whose data we reviewed when analyzing a decade of samples in Spain is what allows for such detections in a matter of days.
